Thorn has a single phase, with multiple parts.

Thorn spawns Spirit Animals within the arena. Players must kill enough of these animals to spawn the Spirit Bear. Players can view the progress by looking for the sea lantern ring around the arena.
Spirit Bears are strong Mini-Boss enemies that spawn during the Catacombs Floor IV Thorn boss fight. They appear after 50 Spirit Animals have been defeated, and only 1 exists at a time. When defeated, they drop a Spirit Bow (not to be confused with the Spirit Bow weapon), a single-use bow which is the only weapon that can damage Thorn. In the Master Mode Catacombs Floor IV, Spirit Bears spawn in groups of 3 instead, and only the last one killed drops the Spirit Bow.
Players must successfully hit Thorn four times to finish the fight.

Master Mode Changes
- Thorn now has 6 Health instead of 4 Health.
- 3 Spirit Bears are spawned instead of 1.
- Only the last Spirit Bear killed drops the Spirit Bow.

Trivia
- Thorn's quote "How one dies shows how one lived" is taken from the League of Legends character Kindred.
- The second half of this same quote, "And it was your masterpiece," seems to be inspired by Jhin, another League of Legends champion.
- The remains of Thorn can be seen in the Catacombs Floor V AND Master Mode Floor V Boss room, with Livid referencing them in his dialogue before combat begins. This dialogue suggests that Thorn and Livid did not have a particularly close relationship.
- Prior to the eventual release of Catacombs Floor VII, Thorn was the only non-humanoid BOSS within The Catacombs.
- Thorn has hidden dialogue when the party defeats him. These lines never appear in game due to the crowd dialogue overridding it:
