The Watcher is the BOSS of the Catacombs Entrance and a Mini-Boss in every following floor's Blood Room that the players have to defeat to enter the boss fight.
Fight Mechanics
Within the Catacombs, The Watcher spawns as soon as the Blood Room is opened with the Blood Key. They begin with some dialogue and then start spawning their undeads that he picks from the walls in the blood room. Their heads will travel to the center of the room where they will spawn.
Once all the summoned mobs are killed, the portal to the boss fight will be opened, or if you are in the Entrance floor, the run will be completed. The Watcher itself cannot be damaged, and if you try to do so, you will take Damage from its laser attack.
There are several things to note about higher floors:
- In Floor II and above, certain mobs now have special behaviors, as listed below.
- In Floor III and above, a version of Bonzo can spawn in.
- In Floor IV and above, two
Watchful Eyes spawn, which are invulnerable but damage players standing above
Y=64
for 25% of their maxHealth as
True Damage per second. Additionally, a version of Scarf can spawn in, replacing Bonzo.
- In Floor V and above, the undeads spawned can have one of six modifiers, or none at all. Each of them spawn with the same modifier, and the Watcher announces what type of modifier will be in the dialogue.

- In Floor VI and above, a version of Livid can spawn in, instead of Scarf or Bonzo. These three mobs will also drop
Livid Fragments,
Scarf Fragments, or
Bonzo Fragments respectively, 50% of the time. The Watcher also no longer tells the player what modifier the undeads have.
- In Floor VII, one of four giants from the Sadan fight (L.A.S.R., The Diamond Giant, Jolly Pink Giant, or Bigfoot) will always spawn in as the first mob. They will drop their respective Precursor Relics 50% of the time (
L.A.S.R.'s Eye,
Diamante's Handle,
Jolly Pink Rock, or
Bigfoot's Lasso).
Watcher Undeads
The stats shown are the base stats before they are modified. Mobs can spawn anywhere between -2,000 to +7,000 Health after modifiers are applied. Minibosses will not spawn when other minibosses spawn.

They have a 0.1% chance to drop a Beating Heart in the Entrance, Floor I, and Floor II, and a 0.2% chance to do so on higher floors.

Trivia
- Players that die while the blood room is open, or that were dead when it was opened are put on the wall and are respawned as a Wandering Soul that attacks players. If the player has not been respawned, and their respective Wandering Soul is killed, they will be revived.
- The Watcher is a reference from the game Terraria. The skull of The Watcher is similar to the Demon Eye. Both The Watcher and the Demon Eye drop an item called the Optical Lens.
- The color of The Watcher's eye changes from green all the way to a darker yellow as the player progresses through the Catacombs.
- In the Journal "The Eye", Sadan's interest in necromancy is related to The Watcher.
